Credentials

My credentials are: MS, LCPC (Licensed Clinical Professional Counselor), CADC (Certified Alcohol and Drug Counselor), and NCC (National Counselor Certification).

Your insurance is a contract between you, your employer and the insurance company. I am not a party to that contract unless I am a preferred provider with your insurance company. My requirement is payment in full at the time of your session, and that you be reimbursed by your insurance company. After you determine coverage, please check to see if you have a copay and/or a deductible which will be collected at the time of your appointment.

My fees are generally considered to fall within the acceptable range by most companies, and therefore are covered up to the maximum allowance determined by each carrier.

I accept health insurance coverage and participate in most PPO programs including but not limited to: Aetna Behavioral Health, Anthem Blue Cross Blue Shield/Unicare/HealthLink, Blue Cross Blue Shield PPO, Cigna, Magellan, Managed Health Network, Morneau Shepell, United Healthcare/United Behavioral Health and Value Options. Please contact me if you do not see your insurance company listed.

Not all services are covered benefits. Please check your policy to determine coverage. NOTE: While filing of insurance claims is a courtesy that I extend to my clients, all charges are your responsibility from the date services are started.
